Appendicitis is the most common surgical emergency. It classically presents with periumbilical pain that localizes to right lower quadrant and nausea, vomiting, anorexia, fever. Acute appendicitis may result from an obstructing faecolith or some other mechanical blockage. However, presenting rare misleading upper in initial false-negative diagnosis by physician even negative findings on computed tomography (CT) ultrasound, increasing risk of perforation/abscess formation prolonged hospital stay. This report a case atypical during laparoscopic cholecystectomy where correct was not initially considered. Unusual localization documented several times, otherwise known as classical appendix. Findings acute varied according The position retrocaecal region (65-70%). Other localizations were paraileal, postileal, promontoric, pelvic, subcaecal, paracolic retrocaecal.
